How mg → Units Works
Concentration
Divide the vial's total mg by the water volume in mL to get mg/mL. This is the strength of your solution.
Units
A U-100 insulin syringe holds 100 units per mL. Units = (dose in mg ÷ concentration mg/mL) × 100.
Example: 5 mg in 2 mL = 2.5 mg/mL. A 0.25 mg dose = 0.1 mL = 10 units.
Standard Titration Schedules
- ·Semaglutide: Start at 0.25 mg weekly, escalating to 2.4 mg maintenance (Wegovy schedule).
- ·Tirzepatide: Start at 2.5 mg weekly, escalating to 15 mg maximum maintenance (Zepbound schedule).
- ·Escalation: Doses increase every 4 weeks to build tolerance and reduce GI side effects.
Tip: If a dose is not tolerated, healthcare providers typically delay escalation by 4 weeks rather than skipping steps.
